The patient meets with an expert in the field of lifestyle medicine to establish medical & surgical history, lifestyle evaluation, body biometric analysis including weight, and body fat percentage. Diabetes & cardiovascular risk scores are calculated and the patient will receive a customised feedback form of their current lifestyle practises. 

Patients medical assessment and lifestyle evaluation will be studies and thereafter goals will be set for the program, and ideas, concerns and expectations will be evaluated. 

The patient will  be in contact with the lifestyle expert on a daily basis for the first few weeks so that the expert can meticulously study lifestyle and habit patterns.  Depending on the current habits,  the patient will be provided with behaviour change intervention tools and will be supported to make lifestyle changes. These changes will be adapted to support the patient's current lifestyle, routine and habits. 

 The patient and lifestyle expert will be in contact on a regular basis to implement habits that are beneficial for overall health and wellbeing. The patient will be educated via regular handouts, workshops and seminars. The patient will be emotionally supported during their journey, to ensure that they are making changes in a way that positively impacts health and wellbeing. 

The patient will be provided with tools to ensure long term sustainability. Our overall goals is to ensure the patient is confident to continue carrying out changes independently . We will support the patient through follow up sessions after programme has ended. The patient will become a member of "Sustain Support Community" where he/she has access to life long access and support from Sustain Team.

  • Analysis of patient's food diary, current habits, likes and dislikes 

  • Customised food alterations & recipes

  • Provide a "kitchen consult" and makeover 

  • Use food to ameliorate any existing medical conditions and to prevent the onset of others. 

  • Explore relationship with food and mindfulness 

  • To determine the patient's current exercise regimen, if any

  • Explore struggles, challenges and motivators towards exercise 

  • Establish the patient's "exercise personality" and provide a plan suited to that, that is medically safe.
