Diabetes Self Management

Diabetes has been on a remarkable rise over the last three decades, emulating a rise in obesity, weight gain and unhealthy lifestyles. Over 422 million people worldwide suffer from this disease, yet we somehow brush it under the carpet, accept the medicines we take are “life long” and carry on.


At SUSTAIN, we break out of this comfort zone to educate and empower our patients using the internationally accepted, established and well-practiced Diabetes Self Management Classes of the American Association of Diabetes Educators. 

Our classes are run in small groups that are aimed at leaving the patient with a better understanding of diabetes and it’s knock on effects, and are especially geared towards those with diabetes and pre-diabetes.  The classes are based on 7 essential concepts of the American Association of Diabetes Educators; healthy eating, being active, monitoring blood sugars, taking medication, problem solving, reducing risks and healthy coping.  

Using these seven pillars, we strive to support the patients emotionally and help them better manage their condition.  The goal of the Diabetes Self Management classes is for patients are equipped with sufficient knowledge to take control of their health and to optimize their wellbeing with as little medication as possible.


Duration : 2 hours over two consecutive weeks

Continuous Glucose Monitoring 

A continuous glucose monitoring (CGM) system is unlike a single BM reading, as it provides a continuous, dynamic and real time update of your sugars during a 24-hour period. The CGM system captures data every minute and checks your blood sugar every 15 minutes for a period of ten days.

How Does It Work?

A small sensor, size of a coin is inserted onto your upper forearm. This sensor contains a small filament that will remain beneath your skin, where it measures blood glucose. You will be given a hand held reader which can be put to the sensor for you to see your blood sugar readings through out the day

How is this helpful?

A CGM system provides a great deal of insight into how your daily life affects your blood sugar levels. Careful analysis of blood sugars will be done; looking especially for patterns, high sugars, episodes of hypoglycemia, the effect of foods, stress, exercise on the blood sugar readings.  Problem areas will be identified and solutions will be generated in order to bring the blood sugar readings to acceptable ranges.

Weight Management Program

Over 1.9 billion people worldwide are overweight or obese and the rates are rising exponentially, even in what appears to be the ‘health conscious era’.  Maybe it’s time to realize that the “diet epidemic” is something that has definitely not helped in our favor over the last few decades and has just contributed to this rise in obesity. 

Moreover, weight loss can be a daunting task, especially if you’ve tried and failed multiple times without reaching your goals. Yet, the need for weight loss, especially in those who are obese or overweight is tremendous.

Losing even a small percentage of your body weight can have so many health benefits which are not limited to increase energy levels, better mood, less chance of diabetes and cardiovascular disease, better bone health and much more.

We focus on the medical aspects of weight and wellbeing, physiotherapy and nutrition  in order to reach individual patient objectives.  By exploring current habits, fears, struggles and barriers, we formulate and strategize novel food plans.


We will equip the patient with tools, lifestyle modifications, exercise regimens and medically manage all areas of weight including comorbidities such as endocrine conditions such as diabetes and cardiovascular disease.We achieve this through a tailor made medically supervised 12-week program that uses a multidisciplinary approach. During this time, we remain in close 1 to 1 contact with the patient to ensure that they’re supported during this entire journey.

Intensive Therapeutic Lifestyle Change Program 

With the rise in non-communicable diseases, it is becoming so much more vital for healthy lifestyles to begin at younger ages in order to prevent these chronic diseases. More and more, we are faced with issues like obesity, diabetes, hormonal disturbances, poor bone quality, sleep issues at a younger age, all of which are interrelated with our lifestyle and current habits.


With the shift towards preventative medicine, this transformational 12-week program aims to explore each facet of your life, inform, educate, guide and provide you with tools for achievable and sustainable changes.  


Duration : 3 months

Mindful Eating Program 

This program is based on the well known US  “Am I Hungry?” Mindful Eating Program. This program is ideal for you if you want to break away from rigid diet plans, yoyo dieting, binge eating or restrictive eating and simply want to start reconnecting with your body. Mindful eating encompasses: 


  • Awareness of your physical and emotional cues

  • Recognition of your non-hunger triggers for eating

  • Learning to meet your other needs in more effective ways than eating

  • Choosing food for both enjoyment and nourishment

  • Eating for optimal satisfaction and satiety

  • Using the fuel you’ve consumed to live the vibrant life you crave
